Despite arriving at the New Delhi airport well in advance of his 2:50 p.m. departure to Karipur, he was unable to board his flight, forcing a last-minute scramble to secure a later seat to Kannur.

The incident has triggered a swift, albeit embarrassed, response from the state administration. Aswathy Sreenivas, the Additional Resident Commissioner at Kerala House, confirmed on Sunday that a formal investigation is underway. While the protocol team had initiated the necessary paperwork, the execution proved fatal to the itinerary.

The breakdown in communication

According to preliminary findings, the oversight was not a failure to act, but a failure to confirm. The protocol officials at Kerala House, who are tasked with managing the movements of state dignitaries, did send an email to the airline regarding Mr. Vijayan’s travel. However, the process stopped there.

"The communication should have been followed up. That is where the lapse occurred," Ms. Sreenivas explained, noting that the team failed to secure a mandatory acknowledgement from the carrier. Because the airline never received or processed the update correctly, the reservation did not align with the passenger’s presence at the VIP lounge. The office is now escalating the issue to the airline’s corporate headquarters to understand why the digital hand-off failed so completely.

This incident serves as a stark reminder of the "last-mile" risk in bureaucratic coordination. While state protocols are designed to be seamless, they rely heavily on antiquated email-and-response chains that are prone to human error.
